All surviving family members must fill out a Small Estate Affidavit when the primary and contingent beneficiaries are deceased.  Or, if the primary beneficiary is deceased and there is no named beneficiary on a life insurance policy.  Some insurance companies have their own particular Small Estate Affidavits.  Others will accept a “form” that your funding company can provide. The affidavit must be signed, dated and notarized.  Insurance assignment situations can get confusing and the insurance companies take a very close look at assignments where named beneficiaries are deceased.  Be careful and make sure you have an experienced funeral funding company quarterbacking your life insurance assignments.
